Research portends that approximately 50% of rural persons living with HIV/AIDS fail to adhere consistently to highly active antiretroviral therapy (HAART). The current study conducted a randomized clinical trial to determine if a telephone-administered, single session of Motivational Interviewing (MI) can improve adherence to HAART in rural persons living with HIV/AIDS. Participants (N = 42) were randomized to either an adherence improvement intervention condition (n = 21) or a self-monitoring, comparison condition (n = 21). Changes in Self-reported medication adherence on “dose adherence” and “schedule adherence” were the primary outcome measures and were assessed through self-administered medication diaries over 5 weeks. Results indicated that medication diaries improved dose adherence and schedule adherence for both groups. No differences were found in self-reported medication adherence between groups. A telephone-administered, single session of MI does not increase adherence to HAART in rural persons living with HIV/AIDS.
